KSHSAA PRE-PARTICIPATION PHYSICAL EVALUATION FORMS MUST BE FILLED OUT AND SIGNED BY PARENT/GUARDIAN AND ATHLETE AT THE TIME OF PHYSICALS.
Free Physicals will be offered at the Scott County Hospital on Wed. May 12th 5:00-7:00 pm.
Use the main doors for the clinic.
Dr. Rosin and staff will be available for those students Entering High School next year on
Tuesday June 1st and Wed. June 2nd from 6:00-8:00 pm at the High School.
Dr. Rosin will also offer physicals for the Middle School Athletes on Monday June 7th
from 6:00-8:00 pm at the Middle School.
